Operators bring on-the-ground insights into daily challenges, while supervisors understand regulatory requirements, and managers oversee operational workflows. Diverse perspectives ensure no hazard is overlooked.
Second, identify potential forklift-related hazards. Walk through the entire warehouse to spot physical risks like narrow aisles, uneven flooring, obstructed visibility from stacked goods, and improper storage of heavy loads. Also, consider operational hazards such as operator fatigue, inadequate training, and poor communication between forklift teams and pedestrian workers. Document every observed hazard, no matter how minor it seems initially.
Third, evaluate risk severity and likelihood. Use a standardized scoring system to rank each hazard—for example, rate likelihood on a scale of 1 to 5 (rare to frequent) and severity from 1 to 5 (minor injury to fatal incident). Multiply these scores to get a risk priority number (RPN). High RPN hazards, like unmarked pedestrian zones near high-traffic forklift routes, require immediate attention.
Fourth, develop and implement control measures. For high-risk hazards, prioritize elimination or substitution where possible—such as widening narrow aisles or installing clear visibility barriers. For risks that can’t be eliminated, use engineering controls like speed limiters on forklifts or administrative controls including mandatory rest breaks for operators. Ensure all operators receive updated training on new safety protocols.
Fifth, monitor and review the assessment regularly. Schedule quarterly follow-ups to check if control measures are working effectively. Track near-miss incidents and adjust the assessment as warehouse layouts, workflows, or equipment change. Keep detailed records of all assessments and updates to maintain compliance with occupational safety regulations.
